TO REQUEST A PARAMEDIC REPORT BY MAIL OR TELEPHONE

If you are requesting a paramedic rescue report and you are the patient, please include:

(Report Request Form)        

1. Name of patient.
2. A copy of picture identification.
3. Date of the incident.
4. Approximate time of day.
5. Address and city of the incident. If you do not know the address you will need to include the cross streets.
6. The responding fire station number, if known.

If you are not the patient, you must obtain a release signed by the patient. If you do not have a release signed by the patient, you must subpoena the report. Please contact the subpoenas office at 323 881-3039.

All requests will take up to 10 business days to process.

ALL REQUESTS WITHOUT REQUIRED INFORMATION WILL BE RETURNED TO SENDER. If you do not have the necessary incident information you may contact the fire station that responded to the incident.
