To request incident or Emergency Medical Services (EMS) reports, please download and complete the “Information Request Form.” At minimum, the date, location and approximate time of the incident must be included. Submit the printed form by mail or in person along with a $15 check or money order made payable to “County of Los Angeles Fire Department” to:

The form may be submitted in person during the following Subpoena & Records Office hours:
Monday and Wednesday 9:00 a.m. to 4:00 p.m. (closed from noon to 1:00 p.m.)
Tuesday and Thursday 9:00 a.m. to Noon
